By entering the In A Day Smile Free Smile Makeover Giveaway, you agree to the following terms and conditions:

1. Eligibility

• Must be a legal resident of Florida, U.S.
• Must be 18 years or older at the time of entry.
• Must be willing and able to travel to Ft. Lauderdale, FL, at your own expense for treatment and all necessary follow-up visits.
• Must be comfortable being filmed and featured in promotional content, including photos, videos, and testimonials.

2. Entry Requirements

• Entries must be submitted through the official entry form.
• Only one entry per person is allowed. Multiple entries will be disqualified.
• All information provided must be truthful. False or misleading information may result in disqualification.

3. Selection Process

• The winner will be chosen based on eligibility, personal story, and need, as determined by Taskonak and the In A Day Smile team.
• Submission does not guarantee The final decision is at the sole discretion of the In A Day Smile team.
• The selected recipient will be notified via phone and/or email. If the recipient does not respond within 72 hours, another applicant may be chosen.

4. Treatment Details

• The winner will receive a full smile makeover with Taskonak at In A Day Smile in Ft. Lauderdale, FL.
• The specific treatment plan will be determined by Dr. Taskonak based on the recipient’s dental condition.
• The recipient must comply with all pre-operative and post-operative care instructions provided by the In A Day Smile team.
• The treatment is non-transferable and cannot be exchanged for cash or other services.

5. Media & Publicity Release

• By entering, you grant In A Day Smile permission to use your name, story, images, and videos for promotional purposes, including but not limited to social media, website content, advertisements, and other marketing materials.
• The winner agrees to participate in before-and-after photos, video documentation, and interviews as part of the giveaway promotion.

6. Liability & Disclaimer

• The recipient understands that all travel and lodging expenses related to treatment and follow-ups are their own responsibility.

7. Agreement & Acknowledgment

By submitting an entry, you agree to these terms and conditions in full and acknowledge that all decisions made by In A Day Smile are final.
